Job Summary:
The role involves orienting individuals and communities regarding the availability of healthcare programs, such as Medi-Cal, for uninsured persons.
The aim is to assist eligible parties with the application process, conduct timely follow-ups, and overcome any barriers to healthcare coverage.
Additionally, the role involves educating families and individuals about the importance of preventive health, as well as the retention and utilization of benefits.
As directed, the job also requires attending events, giving presentations, and conducting one-on-one orientation on Medi-Cal benefits.
Essential Duties and Responsibilities:
Administrative Functions:
Provides presentations to the community on topics such as (Presumptive Eligibility Programs, Family PACT, Medi-Cal, and Covered California) available through DAP Health clinics.
Distribute flyers, brochures, and any necessary documentation before or during each event.
Provides outreach and support to new and existing Medi-Cal recipients in the community.
Establishes positive collaborative relationships with appropriate community agencies.
Networks and collaborates with community partners and internal departments, such as Community Health, to recruit potential Medi-Cal recipients.
Participates in community events such as enrollment, health fairs, food drives, back to school, resources fairs, mobile medical events, and other events as assigned.
Thoroughly assess the qualifications of low- to moderate-income individuals and families to apply for Medi-Cal and other publicly funded programs.
Assists uninsured individuals and families by providing Medi-Cal application assistance.
Accurately document each assistance provided in the electronic health record.
Assists with utilization and retention of benefits.
Provides extensive follow-up/case management to confirm Medi-Cal enrollment and to ensure utilization of benefits.
Communicate with County eligibility workers to identify interventions and/or troubleshoot barriers to benefit usage.
Facilitate access to healthcare services by informing applicants of the benefits and services available to them.
Assists patients in understanding preventive health and facilitates coordination of appointments within the organization.
Assists individuals and families with health plan enrollment or transfer of primary physician.
Meets monthly targets as assigned.
Collects data on uninsured contacts for follow-up.
Provides weekly and monthly reports of activities completed as requested by management.
Attends meetings and training as assigned.
Qualifications:
Experience conducting outreach in underserved populations
Experience with coordinating and participating in outreach events
Experience with community service, health, social service sector, or other relevant work
Knowledgeable in available government programs for healthcare services
