*Job Summary:* As a Care Coordinator, you will be instrumental in assisting with the exceptional care to our clients and caregivers.
Your responsibilities will include assistance with updating care plans, communicating effectively with clients and caregivers, and handling administrative duties.
Position is mostly remote however a few days per week will be office and travel to Client's Location.
Prefer Local to Murrieta, Temecula.

*Qualifications*
* Bilingual proficiency in English and Spanish is a plus.
* Minimum of 2 years of experience in Senior Home Care or healthcare.
* Associate degree or relevant experience in the field.
* Proficiency in software systems, preferably WellSky, and other healthcare industry-related software, including Microsoft Office.
* Minimum typing speed of 40 words per minute.
* Must pass an FBI/DOJ live-scan fingerprinting background check.
*Responsibilities*
* Onboarding and Orientation: Collaborate with the management team to guarantee a smooth transition of new hires from onboarding to orientation through their critical first 90 days.
* Ensure employee files are complete and enter the appropriate information into the scheduling system.
* Work with Care Manager to create, update and manage individualized care plans based on client needs and preferences.
* Regularly communicate with clients and their families to understand and address their care requirements.
* Communicate and review pre-shift reviews with Client and Caregiver to ensure they are well-informed about the client's needs and preferences.
* Communicate and review post-shift reviews with Client.
Report and Collaborate with Care Manager for changes in Care Plan and other areas.
* Conduct regular follow-ups to assess client satisfaction and make necessary adjustments to care plans.
* Licensing & Documentation: Oversee and maintain up-to-date licensing documentation to ensure compliance with industry standards and regulations.
* Maintain accurate documentation on all current and past clients.
* Maintain accurate documentation on all current and past caregivers.
* Assist and manage Caregiver Performance to help maintain attendance and on-time performance among our Caregiving Staff.
* Provide guidance and support to caregivers to enhance the quality of care provided.
*Work Conditions & Schedule:*
* Hybrid Remote position with Office Days and client visits in Murrieta, Temecula and surrounding areas.
Must be local.
* High-speed internet is a must
* A dedicated office space that has minimal noise and distractions.
* Full-time position: Monday – Friday, 7:30 am – 4:30 pm PST.
* On-call duty during off-hours (alternating weekly schedule).
Job Type: Full-time
Pay: $24.
00 - $26.
